By accepting below, I hereby grant Purposefully Home, LLC, Mobility Plus of North Scottsdale and its principals, agents, and other representatives, permission to use my likeness (obtained through my participation in the Workshop) in any and all publications (including websites), without payment or any other consideration. I hereby release and forever discharge Purposefully Home, LLC and Mobility Plus of North Scottsdale from any and all claims, demands, and causes of action which I (or my heirs, representatives, administrators, or others acting on my behalf) may have by reason of this authorization.

Caregivers, join us on Friday, Aug. 26 at 2:00-3:30pm as we discuss safe patient transfers with our friends from Purposefully Home - Dr. Emily Reilly (Occupational Therapist) and Dr. Devon Turnbull (Physical Therapist).

Patient Lift Demonstration

This hour is designed specifically for caregivers (professionals and loved ones) to get hands on experience learning how to properly transfer - keeping both caregiver and their patients safe.

All attendees must register for our Caregiver's Workshop: Patient Transfers by getting a free ticket for a Caregiver Pass. We will limit attendance to 10 caregivers. We will also have a wait list once all 10 spots are taken.
